Treatment Form
The workhorse of the ChiroX4 data set is the main treatment form. The most typical examination and treatment options are available from this screen. The design of this form provides for speedy entry of subjective complaints, objective findings, with adjustments and physical modality treatment options, while greatly reducing the number of screen branches required to fulfill the necessary entry.
Page Tabs: The form shows five different page tabs with the categories of Adjustment, Extremities (for manipulation), Physical Modality, Subjective, and Objective. A typical daily treatment note can be created from these page tabs. Remember, the touch screen design of these forms allow for a quick; yet thorough, entry.
Subjective Entry: One of the first aspects of a daily treatment note is capturing the subjective complaint for the patient. The most common complaints, including several buttons for different types of pain description, are formatted on the Subjective Entry page tab.
Also notice that the anatomy locations are shown on the same page as the complaint. This provides for immediate and precise documentation of the subjective complaint without branching to any sub-forms. Of course, the full array of surface, bone, joint, dermatome, and muscle anatomy forms are available via the "Anatomy" branch buttons throughout the data set.
Objective Findings: The simplified objective finding page tab provides the most common, day to day objective findings. Again, the one page design permits the speedy entry of the findings. This page also has a anatomy location section.
Some of the common findings included on this page are: Decreased ROM, Palpated Misalignment, Muscle Spasm, Muscle Weakness, Muscle Tightness, Hypertonic Musculature, Hypotonic Musculature, Swelling, Short Leg, Antalgic Appearance, Elevated Appearance, and Pain With Pressure.
Of course the full compliment of objective findings are found within the examination sections of every body system form.
Adjustment
The Adjustment page tab of the Treatment Form provides the posting options for the treatment element of the patient encounter. Again, this page is designed for speed of input without sacrificing any of the design elements required to complete the daily S.O.A.P. note.
Type: The first step in posting the adjustment is to select the type. These options include: Standard, Prone, Supine, Extension, Anterior, Side Posture, Posterior to Anterior, Activator, and Gentle.
Keep in mind that this form, like any form within the data set may be easily changed to suit the specific needs of every clinic; however, it is the goal of the development team to have the most popular and complete options available to use, out of the box.
Location: The second function of posting an adjustment note is to select the location. Note that all vertebral locations are listed, again achieving speed of input with thoroughness of required narrative data.
Extremities: This page tab provides the options for posting extremity manipulation.
Physical Modalities: Rounding out the treatment form is the Physical Modalities page tab. Options on this page allow for documentation for all of the following physical modalities: Cold Packs, Diathermy, Electrical Stimulation, Galvanic Current, Hot Packs, Infrared Therapy, Interferential Current, Joint Mobilization, Massage, Mechanical Traction, Microwave Treatment, Myofascial Release, and Ultrasound.
Of course, the anatomy location buttons and the treatment elements for time and different settings are included.
Gonstead Listings and Treatment
Gonstead Listing capture and Treatment forms are also available for the doctors who practice this style of Treatment. The Gonstead Treatment form is designed with the exact format as the previously described treatment forms, with the addition of the Gonstead adjustment page.
Type: The first step in posting the adjustment is to select the type of adjustment being performed. The options are: Sitting, Prone, Supine, Head Drop, Anterior, Side Posture, Knee / Chest, Posterior to Anterior Drop, Activator, and Gentle.
Location: Secondly, the vertebral location is selected. Notice that this form includes all vertebral locations.
Gonstead: Lastly, the Gonstead movements for the particular vertebrae are noted.
Listing Review: Additional forms exist to allow for the initial posting of the listing; as well as the review of the listings at any time during the course of treatment.
Activator
The Activator Treatment forms are available for the doctors who practice this style of Treatment. The Activator form is designed with the same format as the previously described treatment forms, with the addition of two Activator adjustment pages.
Pelvic Deficiency: The first step in posting on the first activator adjustment page is selection of the Pelvic deficiency.
Location: Secondly, the joint and vertebral locations are selected.
Additional Activator: An additional Activator page allows Activator Adjustment selection (left, right and bilateral) to the major areas of: Sacrum, Pelvis and Related; Hip and Related; Knee and Related; Foot, Ankle and Related; Shoulder and Related; Clavicle; Elbow, Wrist and Hand; and TMJ / TMD Syndrome.
