At the core of PracticeStudio's Chiropractic chart screens lie the procedure-specific workflow forms.
Main Treatment Form
The foundation of Chiropractic charting is the main Treatment Form. The most typical examination and treatment options are available from this screen. The design of this form allows you to quickly enter subjective complaints and objective findings, as well as to access adjustments and physical modality treatment options without having to branch off to a large number of additional screens in order to complete your documentation.
Page Tabs
The main Treatment Form contains five page tabs that allow you to navigate to five different screens:
- Adjustment
- Extremities (for manipulation)
- Physical Modality
- Subjective
- Objective
A typical daily treatment note can be created from these page tabs, and PracticeStudio's touch screen design allows for quick yet thorough entry.
Subjective Entry
One of the first aspects of a daily treatment note is capturing the subjective complaint from the patient.
- The most common complaints, including several buttons for different types of pain description, appear on this screen.
- Anatomical regions are also shown on the same page as the complaint. This provides for immediate and precise documentation of the subjective complaint without branching to any sub-forms. You can, however, access the full array of surface, bone, joint, dermatome, and muscle anatomy forms via the "Anatomy" branch buttons throughout the collection of chart screens.
Objective Findings
The simplified objective finding page tab provides options for documenting the most common day-to-day objective findings. This screen also contains an Anatomy Location section. Some of the common findings on this screen include the following:
- Antalgic Appearance
- Decreased ROM
- Elevated Appearance
- Hypertonic Musculature
- Hypotonic Musculature
- Muscle Spasm
- Muscle Tightness
- Muscle Weakness
- Pain with Pressure
- Palpated Misalignment
- Short Leg
- Swelling
A full complement of objective findings can be found within the examination section of each body system form.
Adjustment Forms
The Adjustment page tab of the main Treatment Form provides posting options for the treatment element of the patient encounter.
Type
The first step in posting the adjustment is to select the type. Type options include the following:
- Standard
- Prone
- Supine
- Extension
- Anterior
- Side Posture
- Posterior to Anterior
- Activator
- Gentle
This form, like any form within Chiropractic charting, can easily be customized to suit the specific needs of your clinic; however, with the input of practicing chiropractors, we designed the form to contain the most popular options so that it is usable and intuitive even with no customization.
Location
The second step is to select the location. All vertebral locations are listed and are available for selection.
Extremities
The Extremities page tab provides options for documenting extremity manipulation.
Physical Modality
This page tab allows for documentation of the following physical modalities (including Anatomy Location and treatment elements for time and different settings):

- Diathermy

- Galvanic Current
- Hot Packs
- Infrared Therapy
- Interferential Current
- Joint Mobilization

Gonstead Treatment Form
The Gonstead Treatment form is designed with the same format as the previously described treatment forms, with the addition of the Gonstead adjustment page.
Type
Type options include the following:
- Sitting
- Prone

- Head Drop
- Anterior
- Side Posture
- Knee/Chest
- Posterior to Anterior Drop
- Activator
- Gentle
Location
All vertebral locations are listed and are available for selection.
Gonstead
Gonstead movements for the selected vertebra(e) can be noted.
Listing Review
Additional forms exist to allow for initial posting of the listing, as well as for the review of listings at any time during the course of treatment.
Activator Treatment Form
The Activator form is designed with the same format as the previously described treatment forms, with the addition of two Activator adjustment pages.
Pelvic Deficiency
The first Activator adjustment page contains options for documenting pelvic deficiency.
Location
Joint and vertebral locations are then selected.
Additional Activator
An additional Activator page allows for Activator Adjustment selection (left, right and bilateral) of the major areas of each of the following:
- Clavicle
- Elbow, Wrist and Hand
- Foot, Ankle and Related
- Hip and Related
- Knee and Related
- Sacrum, Pelvis and Related
- Shoulder and Related
- TMJ/TMD Syndrome
